Call Us

To schedule an appointment, you may call either our Woodbridge or Stafford office.  Our offices are open Monday through Friday from 8am - 5pm.

Email Us

You may email  us to schedule an appointment.  We will make every effort to accommodate your request, however, please provide us with more than one date and time.  If you do not receive a response within 24 hours, please call our office.
    
Cancel an Appointment
   
If you need to cancel your appointment, please call our office.  If you call after business hours, you may leave a message on our non-emergency line.  Cancellations must be made at least 24 hours in advance.  Late cancellations may be charged a cancellation fee.

Patient Forms
   
Prior to your appointment, you may download our Patient Registration form, a History and Physical form, a Verbal Consent Form, an HIV Consent form, our Financial Policies and Procedures  and our HIPAA Privacy Notice. 

Please fill out the forms completely and bring the following with you to your appointment: 

• Patient Registration Form
• History and Physical Form
• Verbal Consent Form
• Notice of Deemed Consent/HIV Form
• Your Insurance Card (if you have insurance)
• Your copay/payment:  We accept cash, checks, Visa, Mastercard, Discovery, American Express and Debit Cards
  

Keep the Financial Policies and Procedures along with the HIPAA Privacy Notice for your records at home.
