You may also call our prescription line to request a new prescription or a refill prescription. 

Prescriptions are filled within 48 hours of your request.  The required information for prescription refills are:  your name, your phone number, the name and dosage of the prescription you require, and the name and phone number of the pharmacy that you use.  You also have the option to stop by the office and pick up your prescription.  There may be a prescription refill fee in some cases.  We request that all non-emergency calls concerning new or refill prescriptions be addressed during regular office hours.

                                                                                Medical Records Requests

If you require a copy of your medical records, it is our office policy to follow VA Code [§32.1-127.1:03(J)] which states "If an individual requests a copy of her health record from a healthcare entity, the healthcare entity may impose a reasonable cost-based fee, which shall include only the cost of supplies for and labor of copying the requested information (and) postage when the individual requests that such information be mailed." 

Records provided to persons other than the individual, such as authorized attorneys and insurers, which will be continued to be governed by VA Code [§8.01-413(B)] which states that records may be copied within 15 days for a charge not to exceed fifty cents per page for the first 50 pages and twenty-five cents for each additional page and a fee not to exceed ten dollars for searching, handling and mailing the records.  There is also a charge for postage and shipping and handling.  All medical offices should follow their own state's particular law regarding records requests. 

In order to receive a copy of your records, you must make your request in writing.  You may obtain the form from our office or on-line.  Please fill out the form completely.  If you would like to receive a copy of your medical records from our office, click here.  We also have a Medical Records Request form available to you if you would like to request that your records be sent to us from another physician or hospital.  Please fax or send your request to our Woodbridge office.
